I can't answer for other areas, but in Buckinghamshire I emailed Bucks CC Admissions requesting a copy of the invigilators report for the 11+ transfer test, giving date of test/school name and within 3 days have received a copy of the invigilators report. Very impressed with the quick response.
The report is just a photocopy of the invigilators comments, i.e. it lists the disruptions in the test (CD broke midway through, child started crying, another child went to the toilet) - all names have been removed so no identification can be made.
