Slough schools- Appeal

I read through a couple of posts here explaining not to write the whole matter in INITIAL appeal form and wait for another form to be send by a school. I contacted the school and I was told to download one from their site and send it off, the site also says their last date is 15 April. It means that there isn't any initial forms and we have to write everything in this form.
Secondly, in our case, my DD was short by 2 marks so should I write only stage 1 related to her academics, OR I have to write stage 2 extenuating reasons same time as the grammar schools are always full. I guess I got only 1 chance to put everything down on the paper now. Pls advise.
