Who gives the Grammar Schools his scores?

At last the preferences form is in!
Amoung our 4 we have named a Superselective and a part Superselective - does anyone know for sure how the admissions teams for these schools are given our sons scores, so that they can consider our application? Is it a direct request to the primary school from the Grammar school, or cross referenced information from KCC?
