Exactly - but this information states that the form would be sent with the invitation for testing, but this clearly didn't happen in this instance! And not only was it not attached to the invitation e-mail, but there was no mention of it, either - so unless the parent happened to have read that part of the process hidden away in section 3 of the page on the Bucks website, they had no way of knowing that they were missing a vital form!
