CAF form?

CAF form?

Hi,

Just wanted to check that that the form that needs to be filled out is this...

Just checking as it sounds like the right one, but I noticed it never says "CAF" anywhere.

Also as you go through this form it mentions about providing any details that might help in your application, is it a good idea to mention you got a qualifying score or do they already have this information such that there is no need to state this.

that looks like the online version (which the LEA prefer) - they also refer to the paper version which they label as the CAF (common application form) on the right side of the page you linked to https://www.gloucestershire.gov.uk/medi ... 0-caf.docx" onclick="window.open(this.href);return false;

The LEA should already have the information but I did put my son's rank in the free flow box anyway - it can't do any harm.
