Not reading question properly

My DS constantly fails to read question properly, this is for Maths and English and then making rushed mistakes. What would be your advice please? Thank you

I have had a similar issue with my DS for English, where he did not read the question properly.
I have advised him, for "wordy" question point with the pencil on the words as you read the question, that seems to focus the mind a little.

This is something that works for my DS, may not work for others.

But, be advised this may slow them down a bit.
