Do we have to complete SIF for Wilson's after filling CAF

I have just submitted CAF to local council and received a confirmation email.
However, next to Wilson's school and WCGS a red note was added, it said ' You must complete and return the Supplementary Information Form(SIF) directly to this school' .
I checked both school website, but no SIF was found. Please can anyone give me a link of these SIF forms to the above two schools? ( I doubt SIF is required by Wilson's as it wasn't even mentioned at their website)
