My daughter's school was happy to give me a copy of the report sent to independent schools. It's a single A4 sheet and they don't tailor it to individual schools, so presumably it covers everything likely to be requested.
It details her ability in English, maths and science, including predicted SATs levels, plus general academic progress, effort, behaviour, relationships and extra-curricular involvement. It finishes by recommending her to the school.
Overall it gives a very clear picture of what she's like, even mentioning the fact she's 'experimenting with her handwriting style' - very tactful, I thought, as I'd describe it as barely legible!
