As well as other health conditions and lots of medical results. As compelling as they are, I don't think the panel would want, or need, to see them. The simple headlines from his medical records prove his admission and that he was particularly ill on the day stated, along with the fact that he is terminally ill. It's not dad's medical issue I need to prove, but just that it upset my daughter, and that date was within a week before the test. I hope I've got that right.

After reading the other thread, I've looked again at the email about Selection Review, and I'm not sure whether I include my letter, or write the letter on the form. I haven't actually looked at the form yet, as it asks you to have everything ready before you begin, and I want to put the reports/emails and the extenuating evidence into one PDF for each, as including the head's form will make it to the maximum three.

Can I ask one further question please?
I've got everything together, just polishing the cover letter, but am undecided on whether to send in the Year 4 report.
It has a Star Quality on the top of it, which says she is able to grasp new concepts easily and apply them to different areas of learning with ease. That shc can build on her learning and increase her understanding quickly - all great.
It mentions her "exceptional transcription writing skills", but then, in the next sentence, says her writing is "competent", but could be better with more imagination and flair. It then acknowledges that she is working towards this and it will help her already super comprehension skills. So great, but will they see only the 'competent' part of that (incidentally this teacher told me that her only struggle in the 11+ would be the VR when surprisingly the reverse happened).
The Year 5 I will include anyway, it says that she has worked hard in English and deserves her good progress and that she has developed her own writing style can express her thoughts clearly and fluently. It does have the target of using more ambitious punctuation (something we nailed during lockdown).
Then I have the headteacher acknowledging that during lockdown she'd done the work, hence her now predicted GDS.
So do I include the Year 4 report or not? I'm honing in on the one negative, although it was addressed during Year 5 (and the Y4 teacher states that she knew she working on it then).
Any advice would be very gratefully received, as I kind of just want to submit it now.

I think you could, but that in this instance you would need to spell out the argument.So do I include the Year 4 report or not?

For example:
Please note the progress that has been made in Writing.

Finally there is the update from the headteacher dated 9/12/2020.
(Whether the head's email will be enough to convince the panel, I can't say.)

Thank you so much Etienne, I had begun to write far too much about it in my letter (to the point where it was far more laboured), so I will use your suggestions, as they are far more succinct and articulate.
I noticed I'd mentioned Century as 'evidence', but without context not sure it really adds anything.

I know that I said I had finished asking, well, I was wrong.
Just heard from my daughter's class teacher, she has said that she shows flare for writing and is an avid reader. This was noted at parent's evening and she is being predicted at GDS as she has shown potential in that area.
I will add that and send it too, I guess the panel will be undecided (and it will include my message asking her for advice, as unfortunately to save it to PDF requires the whole thread (I don't think screenshotting it then converting would look right, plus it would then be out of context, as it's obvious she is responding to a question).
Do I need to explain the lower Maths score? She has been GDS all the way through, she has always performed at GDS in Maths at exams, so will they want to know why they are predicting her 111-120, even though she's consistently been GDS. I'm just struggling to know whether to address that, or whether that could be explained by the extenuating circumstances. I worry I'm about to highlight flaws that they may not question if I go down that route, but haven't actually asked if I need to address it before.

111 to 120 is the top she can achieve on the KS2 test.

Yes, 111-120 is a 'scaled score'. It's excellent. Like GDS.
Not to be confused with Bucks transfer test scores (where you need 121 to qualify).
Not to be confused with nationally standardised scores like CATs.
System must have been designed to confuse parents!

Thank you Guest55 and Etienne, sorry I didn't explain clearly, will they question the score of 115 in the Maths component of the STT if she has always been GDS throughout and predicted 111-120?
I've merged the reports/comments/medical evidence into two PDFs, one for academic evidence, one for medical. Each component was named before merging, but I don't know how to put the appendices in. Can I just say 'see attached Year 4 report/headteacher's comments'? They are all in one PDF anyway, I guess they'll get printed out? So I know I'm supposed to put appendix 1 etc, but I cannot figure out how to do so. I will include the covering letter separately.

They will notice that the maths component was below 121, but this will probably be outweighed by the fact that she's been consistently at GDS from Y2 onwards.
I can't help with the system, I'm afraid - not familiar with it.
