Quizzer is, if I recall correctly, a "Home-schooler", so has obviously not got a lot of faith in primary school teachers.

While I am not as negative as Quizzer, I would certainly only take teachers' opinions as part of my assessment of a child's ability for a GS. For example, it is perfectly possible for a teacher not to spot a very able child who is bored by the teaching, so hides their light under a bushel.

Cottagegirl, make your own mind up based on a realistic assessment of your daughter and then work gradually towards the 11+. Three predicted Level 5s sounds fine to me.

There is no need to start formal tutoring until a year (or less in some areas) before the tests.
