Help in how to proceed with the SIF form for ColCHSG

Hi , Can anyone help me in getting and filling the SIF form for Colchester girls school in secondary school admission for Redbridge council.I couldn't find the SIF from the council provided link .I remembered filling it before the 11+ test, so why is the council asking again in listing the schools.Shall I leave it and proceed with other choices.TIA

I had the same problem while filing the application form and i just left it and continued to submit the online application form for secondary school.
