How are students answer papers identified

When they do the exam does each child just write their name or do they have a unique reference number.Can results get muddled??

My son said that the paper already had his details filled in, name DOB and ID number etc, not sure if he meant printed on or if the school had filled out?

I think it's printed. They also had a name tag on a specific seat last year, so no much margin for error, really.
